Assisted Living and Memory Care Residency Information

 

 

If you or a loved one is interested in obtaining more information about one of our buildings, we encourage you to stop in, take a tour and talk with our staff and residents. You are welcome to visit anytime during the day, but if you'd like to speak directly with one of our nurses, we recommend you call ahead to be sure one will be available. You are also welcome to make reservations for lunch, as well.

 

If the decision is made that your loved one would like to become a resident and there is a room available, the next step in the process is to schedule an assessment with one of our Registered Nurses. Assessments are done to gain information about your loved one's life and routines and to make sure the type of services available are appropriate for your loved one's physical and medical needs. Our buildings are all private pay, but there are public funding programs available. We encourage all families to contact their county Aging and Disability Resource Center (ADRC) to get answers regarding public funding and other programs and services offered in your community.

Once a person moves in, our nurses will develop an individualized care plan which identifies any health or social issues the resident may be experiencing. It will also outline the approach that will be used to improve those issues and restore, maintain or improve his or her quality of life. Care plans are updated every six months, giving the resident, family members and staff an opportunity to evaluate the plan's success and make any changes that may be necessary.

 

Apartment Residency Information

 

 

If you or a loved one is interested in obtaining more information about one of our independent apartment buildings, we invite you to stop in, take a tour and visit with the property manager and tenants. Property managers’ hours are normally Monday through Friday from 8 a.m. – 4 p.m. Our apartments have controlled access entryways, and occasionally the property managers do step away from their office; so we would encourage you to call ahead. You and a guest are welcome to make reservations for lunch, as well.

 

Park Place Senior Apartments in Platteville offers a number of units at a discounted rate for income-eligible seniors. Residence in one of these units is dependent on yearly income limits. If you think you may be eligible, please contact our property manager for more information.
